General case or one dimensional propagation? In the latter case the maths
is fairly straightforward (at least, I managed to work out the intensity
correctly from first principles a long time ago).

Sorry, I dont know of any references. I'd be amazed if it wasn't mentioned  
in Beranek.

Sure.

Look up Blokhintzev's pair of papers in JASA, 1946.

If you'd like a turbulent background flow, get Tatarski's book.  I
think it's still available from Dover.
