Edmund Optics offers cone lenses (geometrical shape is a cylinder with
a conical "hat"), but only with a small variety of parameters. Does
anybody know another source?

You could ask Sill Optics
http://www.silloptics.de/
I don't think they have them in the catalogue - but a friend of mine who
has worked there has got on sitting on his desk as a paperweight.
